We accept Medicaid (AHCCCS), Medicare plans, and self-pay. Because accepted plans may change, we recommend contacting our Engagement Center at 602-995-1767 to verify that your specific plan is currently accepted.
At Resilient Health, we are committed to providing services to everyone in our community. We do not deny services based on a person’s ability to pay or because services are covered by Medicare, Medicaid, or the Children’s Health Insurance Program (CHIP). We also do not discriminate based on race, color, sex, national origin, disability, religion, age, or sexual orientation.
The link above also provides additional information on eligibility, how to apply, required documentation, and the overall application process.
You have the right to receive a Good Faith Estimate that outlines the expected costs of your mental health care services and medical services.
Under federal law:
If you do not have insurance, or choose not to use your insurance, you have the right to receive a Good Faith Estimate for the anticipated cost of non-emergency services. You may request a Good Faith Estimate before scheduling services. If you are billed at least $400 more than your Good Faith Estimate, you have the right to dispute the charges. Be sure to keep a copy of your Good Faith Estimate for your records.
